    I registered with your dental surgery a while ago – and have had a number of appointments/ treatments since .
    But I haven’t been since covid struck ! Due to looking after my mother , who had severe dementia/ Alzheimers- I just completely forgot about making another appointment.
    Am I still on your register please ?
